To enroll in a class, please fill-out the below Form - the info collected needs to be transmitted to the Fire Department.
Quick Summary: (Effective December 1, 2016)
There are 2
groups of C of F holders: F-25/F-58 and F-59.
F-59 will only need to take the new Active Shooter/Medical Emergency training course. No CBT, No On-Site.
F-25/F-58 will need to take the new Active Shooter/Medical Emergency training course, and in addition they must take:
- the non-fire course (EAP Director course)
- an EAPD CBT at Metro Tech
- an EAPD On-Site
The above Certificates of Fitness are being replaced with a single certificate of fitness for Fire and Life Safety (FLS) Director.
As mentioned above, we must convey information to the Fire Dept. immediately after the class takes place. As a result, we need to obtain this required information up front. Please prepare this Form, then push the Submit Button - it will come directly to us.

REMS is an FDNY certified school authorized to teach the new, required, Active Shooter/Medical Emergency Preparedness training course.
Administration of the class will be very different from other Fire Department classes.
Schools must provide the students with a school letter.
Said Letter must be on official letterhead, stating the issue date, course title, the applicant’s full name, the certification number on the F-25/F-58/F-59 license and the passing score. The letter will be sealed and stamped.
Schools must provide the FDNY C of F unit a list of students, a copy of the student's C of F card, and the required transfer fee. It's mandatory.
The FDNY will mail a new Certificate of Fitness Card to the student after receiving the required list and payment from REMS.
The new Certificate of Fitness Card for current FS/EAPD holders will be the new F-89 Certificate of Fitness- FIRE AND LIFE SAFETY DIRECTOR.
The new Certificate of Fitness Card for current FSD (F-25/F-58) holders will be the new F-85 Certificate of Fitness- FSD W/ACTIVE SHOOTER PREP.
The F-25, F-58 and F-59 Certificate of Fitness will no longer be accepted by the FDNY on or after 09/04/2017.
